Youth Registration Form

Parents (or youth if 18 ages or older) must complete the annual registration form for permission to travel, medical release, etc. Once completed, please turn in to Eva Sellman, youth director.

Building Use Form

Use this Word document to make a request for building space for a meeting. You can then email the completed form, fax it, or bring it to the church office. (If you need the kitchen, you must ALSO submit a Request to Use Kitchen Facilities form, below)

Use this Word document to make a request to use WCPC's kitchen facilities. You can then email the completed form, fax it, or bring it to the church office, along with your Building Use Form (above). 

Authorization for EFT
(PDF format*)

Ensure the church receives your pledge without you having to remember to write the check! Sign up today for Electronic Funds Transfer—it's free—and you have no more checks to pay for and write. Fill out the "Authorization for EFT", sign it and attach a VOIDED check. Transfers happen every month on the 20th of the month. And it is as easy to stop or change amounts as it is to start the transfers—use the same form. Notices received at the church by the first of the month will be made for that month's transfer. Background Check Forms for Volunteers Working with Children/ Youth 
(PDF format)

Complete these documents if you will be volunteering to work the children or youth. Both forms must be completed, placed in an envelope and sealed, and turned in to the office so that a background check and reference check can be made if you are working with children or youth. This check is to be completed every 2 years. If you have questions, contact the church office.
